Given the growing interest in photobiomodulation as a therapeutic resource for various clinical conditions, intravascular laser irradiation of blood (ILIB) has been investigated as a strategy capable of producing systemic effects through the interaction of light with blood components and vascular structures. This study aims to provide a literature review of the principles, mechanisms of action, and clinical applications of ILIB therapy, as well as its different administration routes. To this end, a narrative review of experimental and clinical studies investigating systemic photobiomodulation was conducted, including both invasive and non-invasive modalities, with emphasis on the anatomical regions used for vascular irradiation. The findings indicate that this therapy may modulate biological processes related to microcirculation, inflammatory responses, oxidative stress, and cellular metabolism, while also demonstrating promising results in various clinical conditions, such as chronic pain, metabolic disorders, vascular disorders, and inflammatory diseases. Non-invasive approaches, including transdermal applications over different arterial regions and mucosal tissues, have also been explored as safe and accessible alternatives. In conclusion, systemic photobiomodulation using the ILIB technique represents a promising therapeutic approach with potential applications across different healthcare fields. However, further clinical studies are required to standardize treatment protocols and strengthen the available scientific evidence.
