Abstract

Background: Approximately 15% of psoriasis (Ps) patients followed by a dermatologist have undiagnosed psoriatic arthritis (PsA). Questionnaires, such as the Psoriatic Arthritis Screening and Evaluation (PASE) tool, can be crucial in identifying these patients. Objective: Evaluate the performance of PASE-P (Portuguese version) as a screening tool for PsA in Ps patients. Methods: A retrospective study using cross-sectional data with Ps patients in a dermatology clinic. Instruments (PASE-P, PASI, DLQI and NAPSI) were applied to all participants. Results: A total of 63 patients were included, of whom 57.1% were men, with an average age of 53.7 years. After applying PASE-P, 33 patients (52.4%) had a CASPAR score of ≥3 and were classified as positive for PsA. Hypertension was the most prevalent comorbidity (33.3%). Median PASE-P scores were 24 (21 – 33) for the CASPAR ≥3 group, and 29 (23 – 36) for the CASPAR <3 group, with no significant difference, p=0.38; qualitative analysis showed a difference in radiographs findings, p<0.01. PASE-P sensitivity and specificity were assessed at various cut points. With threshold scores of 44, 38 and 22, sensitivity and specificity were 12.1% and 80.0%, 18.2% and 76.7%, 69.7% and 20.0%, respectively. PASE-P had an AUC curve of 0.58. Conclusion: PASE-P did not prove to be an adequate isolated tool to screen PsA in a group of Ps, but it can be an ally to identify these patients, especially when additional radiographs variables are included.
