Do diagnóstico ao manejo – Hemorragia gastrointestinal

Introdução: A hemorragia digestiva é definida pelo sangramento nas áreas do trato gastrointestinal, que se apresenta clinicamente com a perda visível de sangue e se manifesta por hematêmese, hematoquezia e melena. Os sinais podem ser variáveis e dependem da origem da hemorragia. Esse sangramento pode ocorrer acima ou abaixo do ângulo de Treitz sendo classificado como Hemorragia Gastrointestinal Alta ou Baixa, ou de algum local de origem desconhecida como Hemorragia Gastrointestinal Obscura. Metodologia: Realizou-se buscas no Medical Literature Analysis and Retrievel System Online (PubMed/Medline), na Biblioteca Virtual em Saúde (BVS) e no Scholar. Após o afunilamento, 30 produções científicas foram selecionadas para a produção desta revisão de literatura. Discussão: A hemorragia digestiva, em grande parte, se manifesta pela hematêmese e hematoquezia. Se a lesão for no terço superior, é mais frequente a hematêmese, e se for no terço inferior, se manifestará em sua maioria pela hematoquezia. As suas principais causas se devem à úlcera gástrica ou péptica, gastrite e uso medicamentoso excessivo na hemorragia alta e à doença diverticular, angiodisplasia e o divertículo de Meckel na hemorragia baixa. O diagnóstico por ser realizado pela endoscopia digestiva alta ou pela colonoscopia associada à outros métodos, tais como, cintilografia ou arteriografia, sendo a endoscopia e arteriografia métodos de diagnóstico e tratamento dessas patologias. Conclusão: O diagnóstico clínico do sangramento gastrointestinal é fundamental para definir intervenção e tratamento específicos. Assim, a endoscopia digestiva alta e a colonoscopia são os exames de escolha para as investigações iniciais. Ademais, em pacientes instáveis a ressuscitação volêmica deve anteceder qualquer investigação.
