A 45-day-old Rottweiler puppy was admitted to the Veterinary Clinic at PUC-Campinas on 26 July 2024, presenting three congenital anomalies: type I anal atresia, preputial hypoplasia, and hypospadias. During the clinical examination, these conditions were diagnosed, and the patient was referred for imaging tests to assess the extent of the malformations, identify possible associated abdominal anomalies, and plan the surgical treatment. Corrective surgeries, including anoplasty and preputioplasty, were performed on the same day, with the anoplasty preceding the preputioplasty due to priority. In the immediate postoperative period, the puppy showed good recovery, eating normally, defecating and urinating appropriately, and remaining alert throughout the recovery period. Postoperative follow-up revealed positive progress, with stitches being removed 21 days after the procedures. The case is presented to highlight the successful surgical approach in treating multiple congenital malformations in a dog puppy.
