Identificar medidas tomográficas de Altura Patelar Tomográfica em Repouso, Contração e Flexão que permitam confirmar o diagnóstico e formular gráficos de probabilidade de luxação patelar em Instabilidades Patelares Potenciais.Métodos:1.705 membros inferiores foram analisados pelas medidas tomográficas baseados no Protocolo da Escola LyonesaEstudo de 921 indivíduos sendo363 (39,4%) do gênero masculino e 558 (60,6%) do gênero feminino com média de idade de 33,7 ± 11,2 anos distribuídos em 4 grupos: Normal – N (n = 87) joelhos de indivíduos assintomáticos para os quais foram tomadas as medidas tomográficas protocolares. Grupo Síndrome Patelar Dolorosa–SPD (n=596) pacientes com clínica de dor ou falseamento e ausência de critérios maiores positivos nas medidas tomográficas. Grupo Instabilidade Patelar Potencial–IPP (n=1.070). A estimativa da diferença entre médias realizada pelo teste t de Student, a diferença entre medianas, para variáveis de distribuição assimétrica realizada pelo teste de Mann-Whitney e a comparação das medidas entre o grupos realizado pela Análise da Variância (ANOVA) e Análise da Variância de Kruskal-Wallis.Curvas ROC foram construídas para estimar os pontos de corte e respectivos índices de sensibilidade e especificidade em cada grupo. A probabilidade de luxação foi estimada por meio de regressão logística univariada. Foi mensurada em 3D sagital da altura patelar em repouso, contração e flexão em todos os grupos. Resultados: Observou-se prevalência do desalinhamento patelar em mulheres (58,7% no grupo IPP e 64,7% no Grupo IPO), A média de idade dos pacientes da amostra foi de 33,6 + 11,2 anos (16 a 72 anos) no grupo IPP e de 27,9 + 13,0 anos (16 a 75 anos) no grupo IPO Quando se correlacionou o grupo normal com os grupos IPP E IPO as três medidas de altura patelar apresentaram índices semelhantes de sensibilidade e especificidade; mas ao se correlacionar os grupos IPP e IPO as três apresentaram baixos índices de sensibilidade e especificidade para o diagnóstico diferencial. Ou seja, bom para dar o diagnóstico, mas não para o prognóstico. A probabilidade de luxação no grupo IPP foi calculada em cada medida: ISR (repouso), ISC (contração) e ISF (flexão) Conclusões: O cálculo da altura patelar tomográfico demonstrou-se eficiente e comprovou que Patela Alta é causa de instabilidade patelar e predispõem a luxação da mesma. Os gráficos de probabilidade desenvolvidos podem ser utilizados como preditivos em pacientes que apesar de instáveis ainda não luxaram suas patelas.
