Background: Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder affecting women of reproductive age, characterized by hormonal imbalances, insulin resistance, and metabolic disturbances. Nutritional interventions, particularly the ketogenic diet (KD), have emerged as promising non-pharmacological strategies for improving these alterations. Objective: This review aimed to analyze the evidence on the effects of the ketogenic diet intervention on metabolic and hormonal levels of adult women with PCOS. Methods: A literature review was conducted using PubMed, ScienceDirect, and Google Scholar databases. A total of 7 clinical studies published between 2020 and 2025 were selected, focusing on clinical trials in women aged 18–45 years, written in English, and assessing metabolic and hormonal outcomes associated with ketogenic dietary interventions. Results: Evidence from clinical trials indicates that the ketogenic diet significantly reduces glucose, insulin, HOMA-IR, total cholesterol, triglycerides, and LDL cholesterol levels, while HDL cholesterol levels tend to increase. Hormonally, KD lowers luteinizing hormone and total testosterone, improving hyperandrogenism and promoting partial ovulatory recovery. Conclusion: The ketogenic diet appears to be an effective nutritional approach for improving insulin sensitivity, lipid metabolism, and hormonal balance in women with PCOS. Its implementation may serve as a supportive strategy to enhance metabolic and reproductive health outcomes in this population.
