Abstract

Dermatofibrosarcoma protuberans (DFSP) is a rare soft tissue sarcoma characterized by slow growth and local aggressiveness. Fibrosarcomatous transformation represents an uncommon subtype associated with more aggressive biological behavior and a higher risk of recurrence. We report the case of a 37-year-old woman presenting with an expansive lesion in the left parietal scalp region, with recent rapid growth after a long-standing course. Magnetic resonance imaging demonstrated a hypervascular extracranial lesion with intense contrast enhancement, diffusion restriction, and increased perfusion, without intracranial extension. Histopathological and immunohistochemical analyses confirmed fibrosarcomatous dermatofibrosarcoma protuberans with diffuse CD34 positivity and elevated Ki-67 proliferative index. This case highlights the importance of magnetic resonance imaging in lesion characterization, assessment of tumor extension, and therapeutic planning.
