The aging population in Brazil is increasing rapidly, posing significant challenges to the healthcare system, particularly in the care of institutionalized older adults. Long-term care facilities for the elderly (LTCFs) house individuals with multiple comorbidities, high medication use, and increased vulnerability to adverse events. This study aimed to analyze the sociodemographic, clinical, and pharmacotherapeutic profiles of female residents in a Brazilian LTCF from March 2024 to March 2025. An observational, cross-sectional, descriptive design was used, based on the review of medical records and clinical documentation. Variables related to health conditions, polypharmacy, the use of potentially inappropriate medications (PIMs), and the occurrence of drug interactions (DIs) were assessed. Results revealed a high prevalence of polypharmacy and PIM use, particularly psychotropic, along with several clinically relevant drug interactions. Biochemical parameters for vitamin D and vitamin B12 were also evaluated, showing a frequent need for supplementation. A significant association was found between metformin use and lower serum vitamin B12 levels, highlighting the importance of continuous laboratory monitoring. The findings indicate that the pharmacotherapy of institutionalized older women requires multiprofessional follow-up, emphasizing periodic medication review, prevention of drug interactions, and ongoing healthcare team education to ensure rational medication use and promote patient safety.
