Diagnóstico Tardio de Aspergilose Pulmonar Invasiva em Paciente com HIV/AIDS: Relato de Caso

Relata-se o caso de um homem de 59 anos, recentemente diagnosticado com HIV em estágio de AIDS avançada (CD4: 55 células/µL), apresentando febre, tosse, dispneia e pancitopenia com evolução de semanas. A tomografia computadorizada de tórax identificou nódulos pulmonares bilaterais. O tratamento empírico inicial para tuberculose pulmonar não resultou em melhora clínica, sendo associado a hepatotoxicidade. Houve deterioração clínica, com suspeita de linfohistiocitose hemofagocítica ou síndrome inflamatória de reconstituição imune (SIRI), tratada com dexametasona endovenosa. A cultura de lavado broncoalveolar revelou Aspergillus Niger, sugerindo aspergilose pulmonar invasiva (API). O paciente foi tratado com anfotericina B lipossomal por 14 dias, seguido de itraconazol por 12 semanas, com resolução clínica. A API em pacientes com HIV/AIDS é frequentemente subdiagnosticada devido à baixa sensibilidade das culturas e à ausência de testes diagnósticos rápidos, destacando a importância da elevada suspeição clínica.
