Hérnia Traumática de Parede Abdominal: Revisão

A hérnia traumática da parede abdominal (HTPA) ainda é um desafio para cirurgiões devido à divergência quanto ao momento da cirurgia, via de acesso cirúrgico, uso de tela, método diagnóstico, lesões associadas e evolução. Objetiva-se relatar um caso de HTPA e realizar revisão de literatura sobre o conhecimento atual dessa patologia de difícil manejo na urgência. Para tanto, procede-se ao relato de caso de hérnia abdominal traumática em paciente hemodinamicamente estável, cujo diagnóstico foi confirmado por tomografia e o tratamento de urgência realizado por via aberta com uso de tela. O caso descrito foi comparado à revisão sistemática de literatura realizada através de busca nas bases de dados Lilacs, IBECS, SCIELO e PubMed, utilizando os termos blunt, abdominal, trauma. Desse modo, observa-se que, dos 44 artigos selecionados, a média de idade foi 36,85 anos, o principal mecanismo foi trauma de guidão (31%), seguido por trauma automobilístico (23%) e esmagamento (21%). As lesões associadas mais encontradas foram fraturas de membros e lesões de delgado e mesentério. Até 15% do diagnóstico passa despercebido ao exame físico. Metade dos pacientes tiveram tratamento precoce (até 24 horas do trauma) por laparotomia exploradora e, em até 42% dos casos, o uso de tela foi dispensado, o que permite concluir que a HTPA pode ser de difícil diagnóstico e de tratamento controverso. Sempre que possível, o reparo cirúrgico na fase aguda é preconizado. O uso de tela pode ser dispensado em lesões pequenas.
