Since the introduction of phenothiazines and butyrophenones, which frequently cause extrapyramidal disorders, the range of applications of biperiden has been extended to include antipsychotic-induced parkinsonism. Common side effects of biperiden are dryness of the mouth, accommodational disturbances, and constipation. We present the case of an elder female patient with schizophrenia who developed a de novo bradycardia after being started on oral biperiden. Though extremely rare as a side effect of biperiden, antimuscarinic agents may cause a dose-related moderate cardiac slowing. Considering its frequent use in psychiatric daily practice we believe that psychiatrists should keep this effect in mind.
