Abordagem da neutropenia febril em adultos no pronto atendimento

Introdução: A neutropenia febril é uma das complicações mais comuns e graves da quimioterapia contra o câncer (ChT). O tratamento quimioterápico suprime o sistema hematopoiético, sendo a neutropenia uma toxicidade hematológica grave, frequentemente limitadora de dose de tratamento. O objetivo desse estudo é reunir informações acerca da neutropenia febril e sua abordagem inicial no paciente adulto no pronto atendimento a fim de reconhecer precocemente a febre neutropênica e iniciar a terapia antibacteriana sistêmica empírica evitando a progressão para uma síndrome de sepse e possivelmente a morte. Métodos: Trata-se de uma revisão narrativa da literatura sobre a abordagem da Neutropenia Febril em adultos no pronto atendimento. Foi feito um levantamento bibliográfico selecionando artigos científicos de revisão e guidelines do tema dos últimos 10 anos. Discussão: Devido à ausência de reação inflamatória, os sinais e sintomas de uma infecção são amenizados e a febre pode ser o único sinal infeccioso. Todos os pacientes devem ser investigados através de uma história cuidadosa, exame físico detalhado, além de estudos laboratoriais, microbiológicos e de imagem. Conclusão: Tendo em vista que a neutropenia febril é considerada uma emergência médica, é fundamental que os profissionais que atuam no pronto atendimento estejam aptos para fazer o diagnóstico precoce, para que o início do tratamento seja feito de forma imediata, a fim de evitar a progressão da doença para uma síndrome de sepse e possivelmente a morte.
