A busca de melhoria no tratamento da dor crônica: o que se sabe sobre a proloterapia

Dor crônica é definida como uma dor recorrente ou persistente que afeta o indivíduo por mais de três meses e está associada a um significativo sofrimento ou perda da qualidade de vida. O tratamento da dor crônica, normalmente se faz com o uso de medicamentos, como os opióides, os quais apresentam diversos efeitos colaterais e quadros de dependência. Sendo assim, se faz necessário a busca por outras alternativas de tratamento como a proloterapia. A proloterapia é o procedimento em que há injeção de dextrose de concentração variada - de 5 até 25% - guiada ou não por ultrassom para um tecido lesado. Seu mecanismo de ação não é completamente elucidado, no entanto, acredita-se que adição de dextrose realiza uma inflamação inicialmente, com posterior remodelação tecidual e melhora da dor. O trabalho teve por finalidade agrupar o conhecimento que se há sobre o uso de proloterapia como tratamento para dor crônica.: Foi realizada uma busca nas bases de dados: PUBMED; SCIELO; Google Acadêmico e LILACS, com as palavras-chave: “Dor crônica” e “Proloterapia” ou “Dextrose”, sendo filtrados somente artigos publicados nos últimos 10 anos, realizados em humanos e com texto completo. Foram encontrados 83 artigos, e após análise do resumo restaram 24. Foi realizada a leitura na integra destes 24 artigos e agrupadas as principais informações, além de seus respectivos resultados. Em suma, 19 dentre os 24 artigos demonstraram eficácia no tratamento com proloterapia, 3 artigos mostraram resultados favorecendo outras opções de tratamento, porém observou-se uma lacuna de conhecimento sobre o mecanismo de ação da terapia e seu tempo de eficácia.
