Hérnias lombares são defeitos na parede abdominal posterolateral, de acometimento raro. O tratamento padrão ouro é a correção cirúrgica com aposição de tela. Trata-se de caso de paciente idosa, com hérnia de Grynfelt bilateral, contendo ureter em um dos lados, sendo esta uma apresentação raríssima. Até o presente momento não foi encontrado em revisão bibliográfica a descrição de hérnia de triângulo lombar superior com conteúdo renal/ureteral. Trata-se de caso raro, com apresentação incomum.
