Leiomiossarcomas são tumores malignos com origem em células mesenquimais, que acometem as extremidades em até 80% dos casos e se disseminam mais comumente pela via hematogênica, principalmente para pulmão. A ressecção em bloco do tumor com preservação de margens é a base do seu tratamento, porém a combinação de cirurgia com radioterapia oferece melhor controle local. Para pacientes com doença irressecável ou metastática, o uso de regimes de quimioterapia ainda é a modalidade mais estudada. O objetivo deste artigo é relatar o caso de um paciente jovem com diagnóstico de leiomiossarcoma de coxa direita ressecado em 1998, que permaneceu em seguimento sem evidência de doença durante 14 anos, quando apresentou recidiva sistêmica multivisceral (vesícula biliar, deltoide esquerdo, pulmões, rins e em assoalho pélvico), tratado com quimioterapia paliativa.
