Objetivo: Relatar caso raro de paciente com abdome agudo obstrutivo portadora de hérnia hiatal tipo IV espontânea. Relato de caso: Idosa de 78 anos acamada e com múltiplas comorbidades, com história e exames sugestivos de abdome agudo obstrutivo, cuja laparotomia demonstrou hérnia hiatal de grande volume contendo porção do estômago e omento. Conclusão: Uma visão multidisciplinar da paciente teria aumentado o índice de suspeição para a patologia e propiciaria diagnóstico e tratamento precoces, visto a presença do quadro atípico de complicação da hérnia hiatal.
